Disconnected care systems: where should a provider start?

When staff repeatedly enter the same information or managers cannot get a clear view across services, the starting point is the work itself. Buying another system before understanding the problem can add complexity.


Follow one important process


Choose a process that matters to staff and management. Walk through it with the people who do the work. Identify where information is created, checked, passed on and entered again. Include the spreadsheets and informal workarounds alongside the main systems.


Separate symptoms from causes


Repeated data entry may reflect disconnected systems, but it may also result from unclear responsibilities, inconsistent information or a process that has grown over time. Establish which issues are technical, operational or a combination of both.


Prioritise a useful outcome


Agree the improvement you want to see. Examples include fewer repeated steps, more consistent information or a clearer management view. Record the current position so that any improvement can be assessed, rather than assumed.


Choose a manageable first change


Assess options against staff effort, dependencies, disruption and the information needed to make the decision. A limited change with a named owner and a clear measure can help establish the next step.


Involve care and operational teams when reviewing the proposed change. Consider access, information handling, training and how work will continue during the transition.
